The practice values the included preventive care at over $550. For anyone attending twice a year as recommended, the cleanings and exams account for most of the membership cost before the 15% discount is applied to anything else.

It is a membership, not insurance. You buy it directly from the practice, so there is no insurance company in the middle: no waiting periods, no annual maximum, no claim forms and no approvals to wait on. It also means it only applies at this practice.

Just a cleaning and a check-up

The membership is almost certainly cheaper, and it puts an emergency exam in place for the rest of the year.

One problem tooth, nothing else

Pay for that treatment and use financing if the amount is awkward. You can always join later if you decide to keep coming back.

Booking for a family

Memberships are per person and the child plan is cheaper. Ask the front desk to work out the total for your household.
